The Program has two funding sources:

• The State of Florida Department of Health (DOH), General Revenue Program
• The State of Florida Department of Health (DOH), and the Agency for Healthcare Administration (AHCA), CF Medicaid Waiver Program

This program helps people to:

• Take responsibility for the management and treatment of CF
• Achieve and maintain self-sufficiency in the home and community
• Reduce or eliminate the financial responsibility incurred by publicly-funded health care programs
• Distribute the cost of CF care among public and private healthcare providers

Services include:

• Medical case management
• Assistance with the rental and/or purchase of medical items and services prescribed by a physician specializing in CF
• Assistance in obtaining health insurance

The Program may serve up to 150 adults with CF per year and is limited to adults diagnosed with CF in cases where medical and support needs are not currently being met. Participants must meet eligibility as defined by the program, and are admitted on a first come, first served basis determined by the date the application process is completed.
